To order an official plaque for a site listed on the New Mexico State Register of Cultural Properties, download and complete an order form below. Please fill out a separate  form for each property you wish to order a plaque for.

The Historical Society of New Mexico, in cooperation with the Historic Preservation Division of the New Mexico Department of Cultural Affairs, coordinates the production of plaques for historic properties listed on the New Mexico State Register of Cultural Properties.

Plaques are 10” x 10” and manufactured from aluminum alloys with flint hardness to give them a scratch resistant surface. The finish is a bronze background with gold colored lettering.

Three types of mounting are available:

  1. Double-Sided Tape – for mounting on any hard, smooth surface.
  2. Blind Tapped (Stud Mount) – includes threaded bolts that require drilling into the mounting surface and securing the plaque with an epoxy or other type of adhesive.
  3. Stucco “Hanging Cleats” or “Z-Brackets” (for an extra charge) – allows for easy removal of the plaque, but it is also the least secure method of mounting.
